After 1871 Berlin, the capital of the new German Empire, became a leading center for medical research. Robert Koch (1843–1910) was a representative leader. He became famous for isolating Bacillus anthracis (1877), the Tuberculosis bacillus (1882) and Vibrio cholerae (1883) and for his development of Koch's postulates. He was awarded the Nobel Prize in Physiology or Medicine in 1905 for his tuberculosis findings. Koch is one of the founders of microbiology, inspiring such major figures as Paul Ehrlich and Gerhard Domagk.[127]
Chris Kresser:  I think that’s like the biggest change we’re going to see, is the nature of this device will change people’s awareness of health, and that’s incredible to think about.  There are so many people who are interested in tech that aren’t necessarily that interested in health.  But due to their interest in tech, they’re going to become interested in health, just because that’s going to be one of the main implementations of the iWatch.  And as you said, there’s going to be such a big community of people developing software.  And what we notice and pay attention to is what we can change.  If we’re not aware of something, we can’t change it.  And that, to me, is the most exciting factor of this new technology. It’s really going to dramatically increase people’s awareness of things—like how many steps they’re taking, and what kind of food they’re eating, and if they’re tracking that, and their heart rate, and how their heart rate variability might correlate to what type of exercise they should be doing that day.  And it’s not just about those kind of specific things that they’re becoming aware of.  It’s that focusing even on a few specific things like that is inevitably going to expand their awareness around all aspects of their health.  So I think it can really be a revolutionary impact.  And I know, as a clinician too, I’m really looking forward to having additional ways that I can both support my patients, by referring them to apps and things that can make implementing some of the recommendations that I give them easier and more practical.  But if I need to collect data for something, some of these devices are going to make that a lot easier and they’re going to be able to send it back to me in a way that’s very actionable for me as a clinician.  It’s a pretty exciting time to be involved in medicine and particularly the evolution of medicine.
Great overview of what it takes to learn and run a Functional Medicine (science-based, systems biology, Integrative) medical practice. I started a Functional Medicine practice in 2009. I wish this book was and approach was available then. There were a lot of growing pains, many of which may have been avoided with the best practices approach outlined in this book. We started out in a fully insurance based practice and at 5 years we were very successful. However, we were also very burnt out. We had talked about creating a model that could be used to help providers make the switch. The bottom line in my experience is that most people can't do that when they're in the trenches seeing patients and learning by trial and error. We never advertised after the initial announcement that we were opening. From there it is was all word of mouth.
Chris Kresser:  So what kind of response are you getting?  I mean, it sounds like, just from the little bit that I’ve heard, that this is really happening at a big level, with The Huffington Post support.  You know, this is getting beyond the typical kind of blog tour that a lot of these summits do.  So what’s been the response in the more mainstream world to the whole concept of functional medicine and doing a summit on this topic?
Susruta, the founding father of Indian medicine, establishes a tradition later enshrined in a classic text, the Susrutasamhita. He identifies 1120 diseases, lists 760 medicinal drugs, and says that the surgeon's equipment amounts to 20 sharp instruments (including knives, scissors, saws and needles) and 101 blunt ones (such as forceps, tubes, levers, hooks and probes).
1953 James Watson and Francis Crick at Cambridge University describe the structure of the DNA molecule. Maurice Wilkins and Rosalind Franklin at King's College in London are also studying DNA. (Wilkins in fact shares Franklin's data with Watson and Crick without her knowledge.) Watson, Crick, and Wilkins share the Nobel Prize for Physiology or Medicine in 1962 (Franklin had died and the Nobel Prize only goes to living recipients).
During the 20th century, large-scale wars were attended with medics and mobile hospital units which developed advanced techniques for healing massive injuries and controlling infections rampant in battlefield conditions. During the Mexican Revolution (1910–1920), General Pancho Villa organized hospital trains for wounded soldiers. Boxcars marked Servicio Sanitario ("sanitary service") were re-purposed as surgical operating theaters and areas for recuperation, and staffed by up to 40 Mexican and U.S. physicians. Severely wounded soldiers were shuttled back to base hospitals.[168] Canadian physician Norman Bethune, M.D. developed a mobile blood-transfusion service for frontline operations in the Spanish Civil War (1936–1939), but ironically, he himself died of blood poisoning.[169] Thousands of scarred troops provided the need for improved prosthetic limbs and expanded techniques in plastic surgery or reconstructive surgery. Those practices were combined to broaden cosmetic surgery and other forms of elective surgery.

In 1953 Jonas Salk announced he had a vaccine for poliomyelitis. A vaccine for measles was discovered in 1963. Meanwhile surgery made great advances. The most difficult surgery was on the brain and the heart. Both of these developed rapidly in the 20th century. A Swede named Rune Elmqvist invented the first implantable pacemaker in 1958. The first heart transplant was performed in 1967 by Christiaan Barnard. The first artificial heart was installed in 1982. The first heart and lung transplant was performed in 1987.

Much of the Egyptian knowledge of physiology undoubtedly derived from their practice of embalming the dead, which allowed them to study the structure of the body. They made some accurate observations about which part of the body was responsible for certain tasks and, despite some inaccuracies due to the limitations of their equipment, they were fine physicians and were unrivalled until the Islamic Golden Age. Ancient Egyptian medicine outstripped both the Romans and Greeks in the level of knowledge and sophistication.
Emil Kraepelin (1856–1926) introduced new medical categories of mental illness, which eventually came into psychiatric usage despite their basis in behavior rather than pathology or underlying cause. Shell shock among frontline soldiers exposed to heavy artillery bombardment was first diagnosed by British Army doctors in 1915. By 1916, similar symptoms were also noted in soldiers not exposed to explosive shocks, leading to questions as to whether the disorder was physical or psychiatric.[166] In the 1920s surrealist opposition to psychiatry was expressed in a number of surrealist publications. In the 1930s several controversial medical practices were introduced including inducing seizures (by electroshock, insulin or other drugs) or cutting parts of the brain apart (leucotomy or lobotomy). Both came into widespread use by psychiatry, but there were grave concerns and much opposition on grounds of basic morality, harmful effects, or misuse.[167]

The history of medicine shows how societies have changed in their approach to illness and disease from ancient times to the present. Early medical traditions include those of Babylon, China, Egypt and India. The Indians introduced the concepts of medical diagnosis, prognosis, and advanced medical ethics. The Hippocratic Oath was written in ancient Greece in the 5th century BCE, and is a direct inspiration for oaths of office that physicians swear upon entry into the profession today. In the Middle Ages, surgical practices inherited from the ancient masters were improved and then systematized in Rogerius's The Practice of Surgery. Universities began systematic training of physicians around 1220 CE in Italy.

1870s Louis Pasteur and Robert Koch establish the germ theory of disease. According to germ theory, a specific disease is caused by a specific organism. Before this discovery, most doctors believe diseases are caused by spontaneous generation. In fact, doctors would perform autopsies on people who died of infectious diseases and then care for living patients without washing their hands, not realizing that they were therefore transmitting the disease.
During the 18th century superstition declined. In 1700 many people believed that scrofula (a form of tubercular infection) could be healed by a monarch's touch. (Scrofula was called the kings evil). Queen Anne (reigned 1702-1714) was the last British monarch to touch for scrofula. Despite the decline of superstition there were still many quacks in the 18th century. Limited medical knowledge meant many people were desperate for a cure. One of the most common treatments, for the wealthy, was bathing in or drinking spa water, which they believed could cure all kinds of illness.

Unfortunately in the 17th century medicine was still handicapped by wrong ideas about the human body. Most doctors still thought that there were four fluids or 'humors' in the body, blood, phlegm, yellow bile and black bile. Illness resulted when you had too much of one humor. Nevertheless during the 17th century a more scientific approach to medicine emerged and some doctors began to question traditional ideas. Apart from Harvey the most famous English doctor of the 17th century was Thomas Sydenham (1624-1689). He is sometimes called the English Hippocrates because he emphasized the importance of carefully observing patients and their symptoms.

medicine has modelled itself after a mechanical physics, deriving from Galileo, Newton, and Descartes.... As a result of assuming this model, medicine is mechanistic, materialistic, reductionistic, linear-causal, and deterministic (capable of precise predictions) in its concepts. It seeks explanations for diseases, or their symptoms, signs, and cause in single, materialistic— i.e., anatomical or structural (e.g., in genes and their products)— changes within the body, wrought directly (linearly), for example, by infectious, toxic, or traumatic agents.[76] p. 510

Japanese physicians immediately recognized the values of X-Rays. They were able to purchase the equipment locally from the Shimadzu Company, which developed, manufactured, marketed, and distributed X-Ray machines after 1900.[149] Japan not only adopted German methods of public health in the home islands, but implemented them in its colonies, especially Korea and Taiwan, and after 1931 in Manchuria.[150] A heavy investment in sanitation resulted in a dramatic increase of life expectancy.[151]
In the Middle Ages learning flourished in Europe. Greek and Roman books, which had been translated into Arabic were now translated into Latin. In the late 11th century a school of medicine was founded in Salerno in Italy. (Women were allowed to study there as well as men). In the 12th century another was founded at Montpellier. In the 13th century more were founded at Bologna, Padua and Paris. Furthermore many students studied medicine in European universities. Medicine became a profession again. However ordinary people could not afford doctors fees. Instead they saw 'wise men' or 'wise women',
Another of Hippocrates's major contributions may be found in his descriptions of the symptomatology, physical findings, surgical treatment and prognosis of thoracic empyema, i.e. suppuration of the lining of the chest cavity. His teachings remain relevant to present-day students of pulmonary medicine and surgery. Hippocrates was the first documented person to practise cardiothoracic surgery, and his findings are still valid.
A number of Greeks speculated that the human body was made up of elements. If they were properly balanced the person was healthy. However if they became unbalanced the person fell ill. Finally Aristotle (384-322 BC) thought the body was made up of four humors or liquids. They were phlegm, blood, yellow bile and black bile. If a person had too much of one humor they fell ill. For instance if a person had a fever he must have too much blood. The treatment was to cut the patient and let him bleed.
From the early nineteenth century, as lay-led lunacy reform movements gained in influence,[157] ever more state governments in the West extended their authority and responsibility over the mentally ill.[158] Small-scale asylums, conceived as instruments to reshape both the mind and behaviour of the disturbed,[159] proliferated across these regions.[160] By the 1830s, moral treatment, together with the asylum itself, became increasingly medicalised[161] and asylum doctors began to establish a distinct medical identity with the establishment in the 1840s of associations for their members in France, Germany, the United Kingdom and America, together with the founding of medico-psychological journals.[23] Medical optimism in the capacity of the asylum to cure insanity soured by the close of the nineteenth century as the growth of the asylum population far outstripped that of the general population.[a][162] Processes of long-term institutional segregation, allowing for the psychiatric conceptualisation of the natural course of mental illness, supported the perspective that the insane were a distinct population, subject to mental pathologies stemming from specific medical causes.[159] As degeneration theory grew in influence from the mid-nineteenth century,[163] heredity was seen as the central causal element in chronic mental illness,[164] and, with national asylum systems overcrowded and insanity apparently undergoing an inexorable rise, the focus of psychiatric therapeutics shifted from a concern with treating the individual to maintaining the racial and biological health of national populations.[165]
Meanwhile In 1875 Robert Koch (1843-1910) isolated the germ that causes anthrax. In 1882 he isolated the germ that causes tuberculosis and in 1883 he isolated the germ that causes cholera in humans. Meanwhile the organism that causes leprosy was discovered in 1879. The germ that causes typhoid was isolated in 1880. The germ that causes diphtheria was discovered in 1882 by Edwin Klebs. In 1884 the germs that cause tetanus and pneumonia were both discovered. Immunization against diphtheria was invented in 1890. A vaccine for typhoid was invented in 1896.

Cartier repaid Dom Agaya by kidnapping him again along with nine other people. By the time of Cartier’s next voyage – to Canada in 1541 – most of the prisoners were dead, but Cartier informed their relatives that they were living in style in France. The scurvy cure did not gain widespread recognition and the disease continued to claim the lives of sailors for more than 200 years.

James Maskell:  Absolutely.  The evolutionary concepts were one of the big reasons why I wanted you to be in there, Chris, because I know you do the Paleo, which is evolutionary in itself.  But also, one of the things that you talk about is how the Paleo diet is something that has needed to change and evolve, and how we’ve evolved to go beyond what our ancestors ate.  I don’t know, maybe for your listeners, they might be interested to just get a snapshot of that.  Because that’s one of the cool things in nutrition that I think that you bring together, is a very sensible approach to eating. I thought that was one of the highlights for the nutrition part of the summit.
It was very difficult for women to become doctors in any field before the 1970s. Elizabeth Blackwell (1821–1910) became the first woman to formally study and practice medicine in the United States. She was a leader in women's medical education. While Blackwell viewed medicine as a means for social and moral reform, her student Mary Putnam Jacobi (1842–1906) focused on curing disease. At a deeper level of disagreement, Blackwell felt that women would succeed in medicine because of their humane female values, but Jacobi believed that women should participate as the equals of men in all medical specialties using identical methods, values and insights.[123] In the Soviet Union although the majority of medical doctors were women, they were paid less than the mostly male factory workers.[124]
Some 200 years later another doctor, Peseshet, was immortalised on a monument in the tomb of her son, Akhet-Hetep (aka Akhethetep), a high priest. Peseshet held the title ‘overseer of female physicians’, suggesting that women doctors weren’t just occasional one-offs. Peseshet herself was either one of them or a director responsible for their organisation and training.
In the Middle Ages the church operated hospitals. In 542 a hospital called the Hotel-Dieu was founded in Lyon, France. Another hospital called the Hotel-Dieu was founded in Paris in 1660. The number of hospitals in western Europe greatly increased from the 12th century. In them monks or nuns cared for the sick as best they could. Meanwhile, during the Middle Ages there were many hospitals in the Byzantine Empire and the Islamic world.
