James Maskell: Well, there’s a reason why I didn’t call it the Functional Medicine Summit, because, I just feel like that is something that’s still arriving, as far the time. But I think everyone really sort of—you know, the cool thing is that people resonate with the concept for different reasons. And, we’ve been on The Huffington Post. We did a whole series of segments on there as part of Arianna Huffington’s Thrive segment. It really fits in with a lot of different areas. So yeah, the response has been great. Bigger medical organizations like George Washington and TEDMED, have all been interested in what we’re doing, because I think people are realizing this is the future of chronic disease management. The Cleveland Clinic announcement about their huge, big functional medicine center, is sort of like a watershed moment in medicine, where it’s saying, “Okay, big conservative organizations also see that this is the future of chronic disease management.” So it seems like the right thing at the right time, and I’m really excited. We came up with the idea for doing this in February and we set the time then. We had no idea that all of this would sort of come together at the same time. But, I’ve learned to just trust the universe and just be happy that things are moving in this direction and other forces are supporting this work.

After 1871 Berlin, the capital of the new German Empire, became a leading center for medical research. Robert Koch (1843–1910) was a representative leader. He became famous for isolating Bacillus anthracis (1877), the Tuberculosis bacillus (1882) and Vibrio cholerae (1883) and for his development of Koch's postulates. He was awarded the Nobel Prize in Physiology or Medicine in 1905 for his tuberculosis findings. Koch is one of the founders of microbiology, inspiring such major figures as Paul Ehrlich and Gerhard Domagk.

510–430 BC – Alcmaeon of Croton scientific anatomic dissections. He studied the optic nerves and the brain, arguing that the brain was the seat of the senses and intelligence. He distinguished veins from the arteries and had at least vague understanding of the circulation of the blood. Variously described by modern scholars as Father of Anatomy; Father of Physiology; Father of Embryology; Father of Psychology; Creator of Psychiatry; Founder of Gynecology; and as the Father of Medicine itself. There is little evidence to support the claims but he is, nonetheless, important.
Temples dedicated to the healer-god Asclepius, known as Asclepieia (Ancient Greek: Ἀσκληπιεῖα, sing. Ἀσκληπιεῖον, 'Asclepieion), functioned as centers of medical advice, prognosis, and healing. At these shrines, patients would enter a dream-like state of induced sleep known as enkoimesis (ἐγκοίμησις) not unlike anesthesia, in which they either received guidance from the deity in a dream or were cured by surgery. Asclepeia provided carefully controlled spaces conducive to healing and fulfilled several of the requirements of institutions created for healing. In the Asclepeion of Epidaurus, three large marble boards dated to 350 BCE preserve the names, case histories, complaints, and cures of about 70 patients who came to the temple with a problem and shed it there. Some of the surgical cures listed, such as the opening of an abdominal abscess or the removal of traumatic foreign material, are realistic enough to have taken place, but with the patient in a state of enkoimesis induced with the help of soporific substances such as opium. Alcmaeon of Croton wrote on medicine between 500 and 450 BCE. He argued that channels linked the sensory organs to the brain, and it is possible that he discovered one type of channel, the optic nerves, by dissection.
In 1478 a book by the Roman doctor Celsus was printed. (The printing press made all books including medical ones much cheaper). The book by Celsus quickly became a standard textbook. However in the early 16th century a man named Theophrastus von Hohenheim (1493-1541) called himself Paracelsus (meaning beyond or surpassing Celsus). He denounced all medical teaching not based on experiment and experience. However traditional ideas on medicine held sway for long afterwards.
This has been a huge acceleration for our organization. James published his book The Evolution of Medicine. 1899 Felix Hoffman develops aspirin (acetyl salicylic acid). The juice from willow tree bark had been used as early as 400 BC to relieve pain. 19th century scientists knew that it was the salicylic acid in the willow that made it work, but it irritated the lining of the mouth and stomach. Hoffman synthesizes acetyl salicylic acid, developing what is now the most widely used medicine in the world.
Founder and currently Executive Editor of Science-Based Medicine Steven Novella, MD is an academic clinical neurologist at the Yale University School of Medicine. He is also the host and producer of the popular weekly science podcast, The Skeptics’ Guide to the Universe, and the author of the NeuroLogicaBlog, a daily blog that covers news and issues in neuroscience, but also general science, scientific skepticism, philosophy of science, critical thinking, and the intersection of science with the media and society. Dr. Novella also has produced two courses with The Great Courses, and published a book on critical thinking - also called The Skeptics Guide to the Universe.
In the paper, Radin explores how frozen colonial pasts operate in the service of biological futures. Radin’s research refigures sample collection, induction and cryogenic suspension as modes of colonial science. Following histories of frozen blood samples collected from indigenous populations in the postwar period, Radin reveals a cryopolitics of “not letting die,” in the service of some future biological development. Radin’s impressive body of work offers unique contributions to the study of Cold War, postcolonial technoscience, genomics, big data, climate history, extinction, science fiction and speculative futures.
The First Viennese School of Medicine, 1750–1800, was led by the Dutchman Gerard van Swieten (1700–1772), who aimed to put medicine on new scientific foundations—promoting unprejudiced clinical observation, botanical and chemical research, and introducing simple but powerful remedies. When the Vienna General Hospital opened in 1784, it at once became the world's largest hospital and physicians acquired a facility that gradually developed into the most important research centre. Progress ended with the Napoleonic wars and the government shutdown in 1819 of all liberal journals and schools; this caused a general return to traditionalism and eclecticism in medicine.
But that is not the whole story. Humans did not at first regard death and disease as natural phenomena. Common maladies, such as colds or constipation, were accepted as part of existence and dealt with by means of such herbal remedies as were available. Serious and disabling diseases, however, were placed in a very different category. These were of supernatural origin. They might be the result of a spell cast upon the victim by some enemy, visitation by a malevolent demon, or the work of an offended god who had either projected some object—a dart, a stone, a worm—into the body of the victim or had abstracted something, usually the soul of the patient. The treatment then applied was to lure the errant soul back to its proper habitat within the body or to extract the evil intruder, be it dart or demon, by counterspells, incantations, potions, suction, or other means.
Throughout the years and across the globe, our understanding of medicine has grown by leaps and bounds. We’ve used plastic and stem cells to build new tracheas for cancer patients. We’ve performed double arm transplants. We’ve even helped a newborn survive a serious heart condition by controlling his internal body temperature and his heart rate with a cold gel and a defibrillator.
The history of medicine shows how societies have changed in their approach to illness and disease from ancient times to the present. Early medical traditions include those of Babylon, China, Egypt and India. The Indians introduced the concepts of medical diagnosis, prognosis, and advanced medical ethics. The Hippocratic Oath was written in ancient Greece in the 5th century BCE, and is a direct inspiration for oaths of office that physicians swear upon entry into the profession today. In the Middle Ages, surgical practices inherited from the ancient masters were improved and then systematized in Rogerius's The Practice of Surgery. Universities began systematic training of physicians around 1220 CE in Italy.
All the way through the Functional Forum and the Evolution of Medicine we've sighted the future of "primary care" to be enhanced through technology, the Functional Medicine operating systems, and coaching for behavior change and this is such a great example. 1656 Experimenting on dogs, English architect Sir Christopher Wren is the first to administer medications intravenously by means of an animal bladder attached to a sharpened quill. Wren also experiments with canine blood transfusions (although safe human blood transfusions only became feasible after Karl Landsteiner develops the ABO blood-typing system in 1900).
Established by Congress in 1959 as the nation's highest scientific honor, the National Medal of Science is a presidential award given to individuals "deserving of special recognition by reason of their outstanding contributions to knowledge in the physical, biological, mathematical or engineering sciences." In 1980, Congress expanded this recognition to include the social and behavioral sciences.
James Maskell: Yeah, absolutely. And it’s cool as well. So in this summit, we have a doctor track as well as a patient track. And in the doctor track, we’re actually talking about some of the ways that this is actually being delivered. And there are ways to deliver functional medicine on insurance. We’re featuring the group visit model in one of the doctor-specific tracks. That’s been very successful at bringing people together, developing a community around groups of people with the same disease. They want accountability. They want support. They want to hear from other people that have the same issues as them. So that’s working and that’s going to be included in the functional center at Cleveland Clinic. And then also health coaches. They’re looking at using different providers together, so you can have higher-cost and lower-cost providers working together. So it’s really exciting. I feel like once we get more and more organizations doing it that are credible, people will work out how to get this done on insurance and how to do this at a bigger scale. The first thing is just the clinical acceptance that’s been a long time coming.
Later in Roman times Galen (130-200 AD) became a famous doctor. At first he worked treating wounded gladiators. Then in 169 AD he was made doctor to Commodus, the Roman Emperor's son. Galen was also a writer and he wrote many books. Galen believed the theory of the four humors. He also believed in treating illness with opposites. So if a patient had a cold Galen gave him something hot like pepper. Galen was also interested in anatomy. Unfortunately by his time dissecting human bodies was forbidden. So Galen had to dissect animal bodies including apes. However animal bodies are not the same as human bodies and so some of Galen's ideas were quite wrong. Unfortunately Galen was a very influential writer. For centuries his writings dominated medicine.
Robert is not a doctor, and what he does is not strictly medicine, but he has created something called the Xpill. It's not a supplement or a prescription, but it seems to have incredible powers to create transformational change. It encompasses looking at placebo response, coaching, group structures, intention setting for patients - you'll find out why this is so interesting to the future of medicine in one of the most fascinating half hours of this podcast we've ever had!
James Maskell: Absolutely. The evolutionary concepts were one of the big reasons why I wanted you to be in there, Chris, because I know you do the Paleo, which is evolutionary in itself. But also, one of the things that you talk about is how the Paleo diet is something that has needed to change and evolve, and how we’ve evolved to go beyond what our ancestors ate. I don’t know, maybe for your listeners, they might be interested to just get a snapshot of that. Because that’s one of the cool things in nutrition that I think that you bring together, is a very sensible approach to eating. I thought that was one of the highlights for the nutrition part of the summit.
On June 1, 2018 a symposium, 100 Years of Women at Yale School of Medicine, commemorated the 100-year anniversary of women at YSM. This daylong event, open to all faculty, students, staff, alumni, and clinicians in the community, was sponsored by the Committee on the Status of Women in Medicine (SWIM), the Minority Organization for Retention & Expansion (MORE), and the Dean’s Office. This event celebrated the contributions of women faculty and alumni from the School of Medicine. The symposium featured speakers, including Naomi Rogers, PhD, Professor in the History of Medicine and of History who discussed the challenges for women in their fields, as well as those encountered on the pathway to finding work-life balance.
When mentioning the Roman influence on the history of medicine, the physician Galen is the most illustrious name. This Greek, granted an expensive education by his merchant father, studied in the medical school at Pergamum and frequented the Aesclepions. In AD161, Galen moved to Rome, where he acted as physician to the gladiators, which allowed him to study physiology and the human body.

The first medical schools were opened in the 9th century, most notably the Schola Medica Salernitana at Salerno in southern Italy. The cosmopolitan influences from Greek, Latin, Arabic, and Hebrew sources gave it an international reputation as the Hippocratic City. Students from wealthy families came for three years of preliminary studies and five of medical studies. The medicine, following the laws of Federico II, that he founded in 1224 the University ad improved the Schola Salernitana, in the period between 1200 and 1400, it had in Sicily (so-called Sicilian Middle Ages) a particular development so much to create a true school of Jewish medicine.
In 1954 Joseph Murray, J. Hartwell Harrison and others accomplished the first kidney transplantation. Transplantations of other organs, such as heart, liver and pancreas, were also introduced during the later 20th century. The first partial face transplant was performed in 2005, and the first full one in 2010. By the end of the 20th century, microtechnology had been used to create tiny robotic devices to assist microsurgery using micro-video and fiber-optic cameras to view internal tissues during surgery with minimally invasive practices.
The operation, Felkin reported, was carried out with the intention of saving both lives. The mother was partially anaesthetised with banana wine. The surgeon also used this wine to wash the surgical site and his own hands, suggesting awareness of the need for infection control measures. He then made a vertical incision, going through the abdominal wall and part of the uterine wall, before further dividing the uterine wall enough to take the baby out. The operation also involved removing the placenta and squeezing the uterus to promote contraction.

In East Semitic cultures, the main medicinal authority was a kind of exorcist-healer known as an āšipu. The profession was generally passed down from father to son and was held in extremely high regard. Of less frequent recourse was another kind of healer known as an asu, who corresponds more closely to a modern physician and treated physical symptoms using primarily folk remedies composed of various herbs, animal products, and minerals, as well as potions, enemas, and ointments or poultices. These physicians, who could be either male or female, also dressed wounds, set limbs, and performed simple surgeries. The ancient Mesopotamians also practiced prophylaxis and took measures to prevent the spread of disease.
As a physician board certified in OBGYN and Integrative Medicine and I was not very savvy to the ways of functional medicine. Then I had problems of my own and I wasn't getting the attention I needed from the allopathic establishment so I went to a functional medicine practitioner and my eyes were opened as a patient and doctor. This book is that wake up call to the rest of the medical establishment that lifestyle and functional medicine is the way that we should be practicing. Maskell is saying what we as physicians need to hear, and it is my hope that we are ready to listen. The book is full of reasons why the new paradigm is not coming, but already here, and in my opinion will eventually be the standard of care. It is also my hope that insurance companies will pick up this book and take heed in the information that we are currently worrying about the end result of disease and need to focus on the power of prevention and gut health. Please read this book and wake up to the call of preventive functional health and be a part of the Evolution of Medicine
